Mukradi (Bestiary 1):

Fearsome centipede-like creatures, mukradi are three-headed predators with a devastating array of breath weapons. Warped by millennia of wild magic storms, living in the Mana Wastes has exposed them to a myriad of conditions that have evolved and mutated mukradi into a perilous foe that adventurers would hope not to encounter. The range of magical exposure from its environment has infused mukradi with elemental powers, granting each of its three heads a separate elemental attack of acid, fire, or electricity that complement their massive maws of piercing teeth.

If razor-sharp teeth and a variety of elemental magic attacks weren’t enough to scare you, the threat doesn’t stop there. Mukradi are seasoned hunters and are both highly agile and resilient. With partitioned anatomy, should a mukradi be confused, paralyzed, slowed, or stunned, it can confine the debilitating effect to a certain portion of its nervous system, ignoring the effect but causing a maw of its choice to go dormant for the effect’s duration instead. Should you score a critical hit against it, the mukradi will go into a spitting rage and its breath weapon recharges and can be used immediately as part of a reaction. And with three heads, a spiny tail, and a long line of legs ready to strike, its thrash attack can send out a flurry of multiple attacks, against multiple enemies, for a flurry of destruction.

The first mukradis are rumored to have spawned in the fevered nightmares of a sleeping demigod from a dimension beyond dreams, who perished as the first mukradi hollowed out its unconscious mind and used its flesh to transition into the mortal realm. This legend is likely nothing more than fancy, but it certainly speaks to the deadly nature of these immense monsters.
